pHealth 2019 Book Review:

Smart mobile systems like micro-systems, smart textiles and implants and sensor-controlled medical devices, together with related networks and cloud services, are important enablers for telemedicine and pervasive health to become the next generation of health services. Social media and gamification have added further to pHealth as an ecosystem. This book presents the proceedings of pHealth 2019, the 16th in a series of international conferences on personalized health, held in Genoa, Italy, from 10 – 12 June 2019. The book includes 1 keynote, 2 of 4 invited talks, 36 oral presentations and 7 poster presentations from a total of 141 international authors. All submissions were critically reviewed by at least two independent experts and a member of the Scientific Program Committee. This process resulted in a full paper rejection rate of more than 30%. This book talks about photoplethysmography (PPG) techniques based on computer-aided data processing. In particular, it presents the results of a co-operative Indo-German project on the topic between Indian Institute of Technology at Chennai and RWTH Aachen University. Measuring system design, experimental details and some preliminary results obtained so far within the framework of this project are presented here. From the investigations carried out so far using the PPG sensors in conjunction with breathing sensors, it has been possible to monitor the 0.125 to 0.15 Hz rhythms in the arterial volumetric changes and to study the influence of breathing on them. These rhythms, which according to medical experts have relevance to psychosomatic conditions e.g. stress or relaxation, can also be addressed to by ancient Indian practices like yoga and meditation. This book presents the results of studying the effects of Indian relaxation techniques like pranayama, meditation, etc. in comparison to western relaxation techniques like autogenic training. So far it has been established that the Indian techniques of relaxation like yoga and meditation are very effective in generating low frequency rhythms in the skin perfusion as monitored by optical sensors. According to medical experts, these low frequency rhythms have a very important bearing on the human physiology and have potential therapeutic implications. This book is meant to provide an overview of the current state-of-knowledge and encourage the next generation of scientists/engineers to carry this work forward, especially on the novel PPG application fields that are of growing importance like pain and stress assessment, detection of peripheral venous saturation and local arterio-venous oxygen consumption as well as contactless space resolved skin perfusion studies with modern camera based PPG technology.

1 Feasibility of the Finger Photoplethysmography to Give Feedback on Chest Compression Rate Book Review:

Purpose: The photoplethysmogram (PPG) recorded using finger pulse oximeter estimates the oxygen saturation. During out-of-hospital cardiac arrest (OHCA) the PPG waveform shows amplitude changes with chest compressions (CC. The aim of this study was to analyze the feasibility of using the finger PPG to provide feedback on CC rate during OHCA.Materials and methods: Records of 46 patients were included, comprising the PPG and compression depth signals extracted from Zoll E-Series defibrillator files. They were acquired by the DFW Center for Resuscitation Research (UTSW, Dallas) and the Emergency Medical Services agencies from the Resuscitation Outcomes Consortiumu2019s Portland site (OHSU). An automatic algorithm analyzed the PPG every 5s and detected useful PPG, presence of CCs and CC-rate. The compression depth signal (acquired by the CPR-pad-z) was used as the reference signal to simultaneously compute the rate and check for presence of CCs. The percentage of useful PPG, and sensitivity (SE)/positive predictive value (PPV) for detecting CCs were computed. The CC-rate error was assessed as mean(standard deviation) and 95% level of agreement (LOA, Bland-Altman). A feedback system on the CC-rate was implemented for the intervals where PPG was available, and evaluated in terms of SE, PPV and specificity (SP) to trigger alarms for CC-rates out of the range 90-120 CC/min.Results: A total of 420 min (9.1u00b17.6 min per episode) were processed. A total of 4971 segments were analyzed, 4319 including CCs with rate 116 (15) CC/min. The PPG signal was not available 17% of the time. The SE/PPV to detect CCs were 86.2/95.4%, and the CC-rate error 2.4 (6.3) CC/min (see figure). The SE/SP/PPV for the feedback system to trigger alarms were 83.6/92.4/85.3% respectively. Conclusions: When available, the finger PPG signal reliably gives feedback on CC rate.

"A new wearable band is developed which uses three Photoplethysmography (PPG) sensors for the purpose of hand gesture recognition (HGR). These sensors are typically used for heart rate estimation and detection of cardiovascular diseases. Heart rate estimates obtained from these sensors are disregarded when the arm is in motion on account of artifacts. This research suggests and demonstrates that these artifacts are repeatable in nature based on the gestures performed. A comparative study is made between the developed band and the Myo Armband which uses surface-Electromyography (s-EMG) for gesture recognition.
