During the last 20 years the development of enzyme histochemical techniques has contributed greatly to knowledge of muscle pathology. However, these and other new methods, such as electron microscopy and immunocytochemistry, have only relatively recently become gener ally available for routine use in histopathology. Muscle biopsy is a long-established technique in clinical practice, having been introduced by Duchenne in 1868 (Arch. Gen. Med. , 11, 5-179). However, the needle method used by Duchenne was not generally adopted, although Shank and Hoagland described a similar technique in 1943 (Science, 98, 592). During this time muscle biopsies required a surgical procedure and this was a considerable disincentive to their use. It was not until Bergstrom (1962; Scand. J. Clin. Lab. Invest. , 14, Suppl. 68) and Edwards (1971; Lancet, ii, 593--6) developed a simple biopsy needle suitable for muscle work in connection with exercise physiology that the advantages of needle muscle biopsies came to be appreciated. Since then, muscle biopsies have become a relatively minor procedure. This has led to the increasing use of muscle biopsy in clinical practice, both for diagnosis and for assessing progress in repeated biopsies during the course of a disorder and its treatment. The full range of enzyme histochemical and ultrastructural histological techniques can be applied to these small biopsies and many of the older histological staining methods can also be used. OBJECTIVES: 1)To correlate the disease course and outcomes of children with juvenile dermatomyositis (JDM) and polymyositis (PMS) with specific features of muscle pathology 2)To ascertain whether children whose health care needs are covered by Medicaid have longer mean time to diagnosis than their peers with private health insurance coverage. METHODS: This is a retrospective cohort analysis of 59 children diagnosed with JDM, PMS or sclerodermatomyositis between 1965 and 1998, followed at the Children's Hospital Medical Center (CHMC) in Cincinnati, OH. Time to diagnosis was measured in months, extending from onset of symptoms to date of diagnosis. Disease course was defined as limited, chronic non-ulcerative or chronic ulcerative [6] and determined by reviewing patients' medical records. All children had disease for at least two years in order to classify their course as limited or chronic and had muscle biopsies performed at CHMC. Features of muscle pathology that were assessed included loss of capillary network, infarct, perifascicular myopathy, direct immunofluorescence (DIF) of vessels and the locale of DIF, i.e., arteries or capillaries. Complications that were evaluated included death, gastrointestinal ulceration, calcinosis, contractures, and muscle atrophy. Data analyses used Wilcoxon rank sum tests for comparing continuous variables not normally distributed, t-tests for comparison of normally distributed variables, Chi-square and Fisher's exact testing for evaluation of associations between disease course, complications and muscle histopathology. A logistic regression model was constructed using DIF arterial staining and infarct as predictor variables and the outcome event, chronic ulcerative disease. RESULTS: Mean and median times to diagnosis were not significantly different between children on Medicaid and those with private health insurance, even when race was eliminated as a potential confounder. DIF arterial staining and infarct were significantly associated with the development of chronic ulcerative disease, death and gastrointestinal ulceration. From the regression model, children with either infarct or DIF arterial staining on biopsy were 8x more likely to develop ulcerative disease, and 60x more likely if they had both findings, over children with neither finding on biopsy. CONCLUSIONS: In this cohort, children on Medicaid did not have longer time to diagnosis than their peers with private health insurance and so were not victims of environmental injustice when accessing medical care. Significant associations between muscle pathology and disease course were found: DIF arterial staining and infarct with chronic ulcerative disease, death and gastrointestinal ulceration. Additional associations between muscle histopathology, disease course and outcomes might have been found with larger sample size and greater statistical power.
